No changes will be made to the information on your I-20.

Use this form if you need:

  • Replacement I-20 (damaged, lost)
  • Travel signature (forgot to get a travel signature before leaving the U.S.)

Do not use this form if you need:

Section 1: Student's Information

Provide email address you check regularly. Notification email will be sent to this email address.

Travel signature is valid for one year
Check the reason for your I-20 request*
For a travel signature, bring your I-20 during the F-1 Walk-in Hours.
If you already know, please indicate the date that you plan to return to the U.S.
Have you changed your address, phone number, and/or personal email address?*

F-1 students are required to report the change within 10 days to the International Education department. Submit the International Student Contact Information Update form.  F-1 specialist will update your information in SEVIS.
